YOUTH Registration and Medical Consent // 2026-27

As with the beginning of every year, we ask parents and caregivers to fill out an all-year waiver for their students to participate in our programming. You will only need to fill it out once per school year.

Please start with YOUR [parent/guardian] information first. Then, under "Family Information", add your child(ren)'s information, as well as any other adults in your home.

We send out weekly emails with program details, event information, and more.

Family Information

Click "Add child" and fill in all the fields for your student. If you have more than one child attending Central YOUTH, please repeat the process for each additional child.
Under medical notes for your child, please include any allergies, dietary restrictions, or behavioural needs.

Click "Add adult" and fill in all the required fields for any other adults in your home.

Waivers and Consent

Please take a moment to carefully review the following waivers/releases.

I hereby authorize any images or video footage of my child/youth (under 19 years of age), in whole or in part, individually or in conjunction with other images and video footage, to be displayed on the Central Baptist Church website and/or other social media: YouTube, Facebook, Twitter, Instagram, and to be used for media purposes including print promotional presentation and marketing campaigns. I also authorize the display and use of any media material created by my child/youth within Central Baptist Church.

I waive rights to privacy and compensation, which I may have in connection with such use of my child’s likeness and first name, including rights to be written copy that may be created in connection with video production, editing, and promotion therewith.

Throughout the program year, our ministry leaders will communicate with your students for the purpose of sharing information, event planning, check-ins, and group conversations.
The rise of social media and other forms of electronic communication has made it easier for both healthy and unhealthy communication to take place.
Therefore, the following guidelines are in place when leaders communicate electronically with youth or children:

- Parents will be informed of all electronic communications between leaders and students.

- All one-to-one communication between students and leaders must be done through a platform that can be documented.
E.g., text message, email, Facebook (including Facebook Messenger), WhatsApp, Google Hangouts.

- All one-to-one communication between students and leaders will be within the hours of 6:00 AM - 10:00 PM.

I give consent for my student(s) to be electronically communicated with by volunteers and staff during the current program year.

I give permission for my child(ren) to participate in the YOUTH programs at Central Baptist Church.

I understand that I will be notified in the case of a medical emergency. However, in the event my child is injured or becomes ill and I cannot be contacted, I authorize for a doctor to be called and for necessary medical services to be provided. I further agree that I have notified Central’s YOUTH Leaders in writing (by way of this form) of health information that would restrict my child’s participation in normal youth activities, or would be necessary in the event of a medical emergency.

I understand that at times, organized Central YOUTH activities involve leaving the church premises. I understand that participating in activities also means assuming responsibility for the risks associated with those activities.

I hereby release Central Baptist Church, its volunteers and employees, from any and all actions and liabilities for injury to person or property arising as a consequence of my youth’s participation in activities relating to the Youth program.
